‘My Health, Our Future – At home’ is a survey you can complete at home to help us make a difference for families at this challenging time.
There’s a survey for young people (aged 11 – 19 or up to 25 for young people with SEND) and also a survey for parents or guardians. Amongst a few other things, we’ll ask about how you’re feeling right now, how your family is coping and how your experiences of accessing local support has changed. We will not ask for your name or contact information.
